Insurance
I have what is called an "out of network" practice, meaning that I am not an in-network provider with any insurance company. Payment is due at the beginning of each of our sessions. If you wish, I will provide you with a form called a “superbill” for each session that you can submit to your insurance company to receive reimbursement directly. In order to do this, your insurance plan must have out-of-network benefits. Not all plans do. Here is what I recommend that you do in order to ensure that you can get reimbursed for our sessions by your insurance company:
Before We Make Our First Appointment
If you wish to use out-of-network benefits, then prior to making our first appointment, you should call your insurance company directly to ask about out-of-network benefits and reimbursement. I recommend that you look on the back of your insurance card and call the number for mental or behavioral health benefits. Ask the following questions to ensure that you understand your benefits:
Do I have out-of-network (OON) mental health benefits; if so, what are they?
Typically OON benefits are quoted as a reimbursed percentage of provider fees. For example, you might be told that your company will cover 60% of an OON provider’s fees.What is the usual and customary or allowable rate that the quoted percentage is based on for the following billing codes: 90791 (intake session), 90834 and 90837 (follow up individual sessions), 90846 and 90847 (follow up family sessions)?
The percentage that you are quoted is based on an internal rate specific to your insurance company called the “usual and customary” or “allowable rate.” This allowable amount varies by insurance company and plan. This rate might be different than my fee, so you need to know it to calculate the reimbursement amount you can expect from your insurance provider. Insurance companies will not tell me what their usual and customary rate is; only clients have access to that information.Do I have an out-of-network deductible? Has my annual deductible been met yet?
On what date will my annual deductible reset?
Is there an annual limit on the number of sessions allowed in a calendar year?

Payment is collected at each session. I accept the following forms of payment:
· All major debit and credit cards
· Health savings accounts
· Flexible spending accounts
· Checks or cash
For your convenience I will keep a card on file and can automatically charge it for you after each session.
